Confidential Mental Health Screening

Mental Health Screening tools are available through Mental Health America of Wisconsin.  Screening tools assist in identifying symptoms related to various mental health conditions and can be accessed at http://www.mentalhealthscreening.org/screening/welcome.asp.  If you complete a Mental Health Screening and it leads you to consider making an appointment with an ACCESS therapist, please bring a copy of your completed Mental Health Screening to your first appointment.

Completion of such screening tool is not a prerequisite to receiving services from ACCESS and any screening assessment would be used only for preliminary services-engagement purposes.
